complete. Completing the
Personal Data Sheet
and
Informed Consent
prior to your first visit with Dr Sullivan will maximize your time with her.
We also ask that you take a few minutes to read through our
HIPPA Privacy Statement and Covid-19 Office Policy
prior to your first appointment.
The Privacy Statement summarizes our respectful handling of any personal health
information obtained during the course of your care with us.
